- When a heartbreaking loss of a loved one hits the Jones family, Jillian travels to Cambridge, Mass., to help them towards a healthier lifestyle. Having been diagnosed with diabetes, Debra lives in fear of leaving her two children without a mother. With only one week to get Debra and her family back on the right track, Jillian shows them various ways to get healthy and have fun with activities that include canoeing, African dancing and ice hockey.
- After suffering a life altering event, mother Ruth May and her three children are in desperate need of re-building their self-esteem and boosting their confidence. In response to the family's strong desire for a positive change in their life, Jillian helps empower them to be successful in their pursuit of healthy living. During Jillian's stay with them she surprises Ruth with a much needed night out to enjoy herself, joins the daughters for a workout with a college soccer team, showing them they can be active without a gym, and through a garage sale, helps rid the family of useless items.
- After Patrick Northern lost his job, he and his family need motivation to get back on the right track. His wife, Kim, has taken on the role of sole breadwinner, causing a serious blow to Patrick's self-confidence. Meanwhile, his 14-year-old twins look to him for direction, and since Patrick lost his drive to get off the couch, so does his family. During Jillian's stay, the family gets active with a step competition, roller-skating, and they throw out a deep fryer that has attributed to their weight gain.
- Dolores Plunkett and her daughter, Coralie, are two of three cultural leaders of their tribe, the Yavapai-Apache Nation. Both want to see a change in their family's unhealthy diet (the result of snacking and convenience), and to help improve the health of their tribe. Jillian travels to Camp Verde, Arizona and joins the family in a traditional Yavapai welcoming ceremony and Apache dancing and shows them that there are still ways to be traditional and stay healthy at the same time by roasting an agave plant and planting corn.
- The Vivio clan is unlike any other family; all six family members are over six feet tall. Two years have past since disaster struck the Vivio family, when a bridge over their creek collapsed, damaging the father Mark's right knee, which led him to discover a threatening heart problem. With injuries preventing him from working, Mark was forced to close his roofing business and take a job as his youngest son Elijah's school bus driver. With the events of the past year, Elijah is also headed towards an unhealthy downward spiral. Jillian must rally the Vivios around Elijah and reverse the damage that has been done.
- Parents Amy and Todd Franklin have been overweight for most of their lives and are in need of a change. After having gastric bypass surgery, Amy lost 100 pounds and since then has regained 50 of those lost pounds due to the stress of losing their family business. Behind closed doors, Todd secretly snacks and believes he is in great shape for someone of his size, yet his two daughters are worried about their father's health. Jillian travels to Huntington Woods, Michigan and gives them a chance to take control of their lives and get back on the right track. Dr. Katja Van Herle also stars.
- Part of a large, close-knit Cuban extended family, David Sr. and Carmen Alvarez have an obstacle in their way that they need to overcome. Their 9-year-old son, David Jr., is already pre-diabetic and on his way to health complications due to his weight. Unfortunately, David Sr. is also overweight, but he fails to see it as a problem that needs to be fixed, which proves to be a bad influence on his son who idolizes him. In order to help the family succeed, Jillian takes the parents on a boat to get them to open up about their health and lack of communication which hopefully will kick-start a new way of life for them. Dr. Katja Van Herle also stars.
