- The PG-13 cut of the film only has what is above this post, but the R-rated Directors cut adds 2 extra minutes of crude sexual references.

Violence & Gore

- A crushed rat is seen on the ground.
- More than twenty people are killed with guns and a missile explosion during a mercenary strike that blows up a jet, several cars, and an airport control tower.
- A woman shoots a man and then pours a flammable liquid on him. A man then throws a lit cigarette onto the man setting him on fire. To top it off, they then set off explosives that blow up a chemical plant around him.
- A woman shoots Spawn with machine guns (he heals) and later he shoots and kills her. He's then blown out a high rise window. Claws from Spawn's suit attach onto the wall and the military then opens fire on him. Spawn's cape vanishes him. Spawn reappears and from the wall of the building, Spawn gets away from the military by a cape flying him away.
- Spawn and the Violator (in the shape of a huge monster) fight, with Spawn shooting the monster many times. It then throws a man across the street, slams Spawn into the side of building and then impales him on a wrought iron fence.
- The Violator shoots hand grenades at Spawn and later the truck driven by the Violator explodes in a huge fireball.
- A man stabs a woman with a knife, killing her. She then comes back to life, kicks Spawn through a door, and then slits open his throat with a knife.
- At the end there's a huge fight between the Violator and Spawn and Cogliostro in Hell and in a house that culminates with a monster's head being cut off and lots of green blood oozing out.
- Scary views of Hell with fires and monsters. A man is impaled on a fence, a woman is stabbed and a man's throat is slit. A monster's head is cut off (lots of green blood). A woman is shot in the head with a bit of blood. Many people are shot and killed, usually with automatic gunfire (but usually without blood).
- Spawn is monstrous looking and sometimes looks like the burn victim he is (bloody and charred). Sometimes people and monsters bleed green blood. A man is blown out of a window. Many fights with punches, throwing people around and kicking. Explosions, threatening with guns.
- Dead body remains are seen in a casket. 1. is a hat, 2. is a necklace with a photo of a couple. 3. is a body bag with a sharred skeleton inside. The skeleton hand grabs Spawn.
- Spawn tries to strangle the clown but Spawn struggles. Three people encounter the two. A satanic weekender puts his hand on Spawn and Spawn throws his body onto another man. Armor forms on Spawn with spikes growing off of his body.
- The clown picks up a plate of pizza with anchovies and sees a lot of maggots and worms crawling on the slices. The clown takes a bite out of the pizza and we hear the maggots scream.
- At a girl's birthday party. A man from hell tries to see his wife but he gets weakened. His body is on the ground and his puppy comes. A girl asks if the man wants some candy and her mother tells her to get away from the man.
- At a dinner restaurant, Spawn crashes down the ceiling window with his red cape and the armor wears off on his face. Spawn strangles a man and throws him onto a table. Spawn grabs the man then a woman suddenly fires.
- Spawn approaches Jess knocking her onto a wall. Jessica tries kicking Spawn then his Belt has a face claw and crushes a part of her leg. Spawn knocks her onto a balcony and shoots her in the head. She crashes onto a table
- The clown farts two times. The clown gets strangled by Spawn and Spawn throws him onto some trash bags. The clown says that he is going to cut him into 50 pieces then a dog pees on him. The clown grabs the dog and uses his big green hand trying to make the dog dead. Spawn shoots the clown and the clown turns to a monster.
- A man with a hat comes to Spawn and Spawn's chains tries chasing him circling around typing Spawn up (harmlessly).

- 1 "s" word, 6 "ass" words (2 used with "hole"), 6 hells, 5 damns, 4 craps, 1 S.O.B., and 1 use each of "Oh God," "Jesus," "Good Lord," "God," and "Christ" as exclamations.

Violence & Gore
- Al Simmons is burned alive. This is shown through shadows, this is how he becomes Spawn.
- The Violator tries to eat Spawn, but Spawn releases his spikes inside the Violator's mouth and then wraps his chains around its neck and decapitates it. The Violator's head then changes back into the Clown's head, lying on the floor in a puddle of ooze.
Frightening & Intense Scenes
- Al Simmons is set on fire; this may be frightening for some viewers.
- Seeing a clown in a cheerleader outfit may be frightening for some viewers.
