Sex & Nudity

  • Skull Kid shakes his buttocks at Link when you zoom at the Moon with the telescope.
  • The Great Fairies are all busty, and wear loin clothes.
  • The "Honey and Darling" characters display an unwavering romance with one another, including PDA.

Violence & Gore

  • Link fights a plethora of battles against various monstrous foes, fighting them with swords, bombs, a bow and arrow and magical weapons. Monsters bleed multi-colored blood when attacked and either explode with a colored fiery haze or disappear when they are killed.
  • If Link should die, he will fall on his face dead.
  • The character of Mikau dies in front of Link after playing his final song.

Profanity

  • No profanity is spoken through-out the game.

Alcohol, Drugs & Smoking

  • A milk-bar exists in the game for the player to visit. A few people appear there over the 3-day cycle drunk. Nothing explicit happens.

Frightening & Intense Scenes

  • There is a subplot where Anju, the innkeeper, is afraid that her fiance has abandoned her. In one scene, she breaks down crying about this when she tells Link about it. Later, she has an argument with her mother, who believes her fiance isn't coming back, and tells her that even if he does, the town will be destroyed by the moon. It's a surprisingly mature storyline, and can upset some players.
  • Whenever Link puts on a mask, he screams in pain and agony. This would frighten a lot of people (especially children).
  • Skull Kid is a creepy character who has a sinister laugh and a loud scream he also has a creepy mask he's very unsettling and creepy.
  • For a game that is rated e for everyone, the game has a notoriously creepy, dark, and sinister atmosphere all throughout. It has been seen as the darkest game in the entire Zelda series.
  • The constantly looming threat of the Moon falling on Termina makes for a very tense gameplay experience.
  • The last six hours of the final day has a BGM tonal shift that, combined with the consistent rumbling of the Moon getting closer and the constantly ringing bells, makes the gaming experience sad and frightening for the player.

Alcohol, Drugs & Smoking

  • There's a Milk Bar in Clock Town. Many theorize that the milk is alcoholic. Ingo even says he got tipsy (drunk) on milk once if you go inside. He even hiccups!
  • Cremia says she's going to let her sister drink milk at night on the final day. The implication is that the milk is alcoholic, and she wants her sister to be in a deep sleep so she won't be awake when she dies.

Frightening & Intense Scenes

  • The game must be finished in three days in game. If you fail, a movie starts in which the moon crashes over the world. It can be very disturbing.
