Sex & Nudity

- Not that much nudity, but the severe rating is appropriate.
- You, as the main character, can strip down to nothing but underwear.
- You can have "sex" with your spouse, but the actual acts aren't shown; only suggestive sounds and sayings can be heard while the screen is dark.
- The brothel house shows women in low-cut clothing revealing cleavage. The head pleasure woman has heavy cleavage.
Violence & Gore

- Blood (and usually a decent amount of it) comes from the enemies when you strike them.
- It is possible to decapitate people. A fountain of blood will stream from their neck with their head spinning on top of the flow. This appears to be played for comical purposes but may be disgusting to some players.
- There is a fist fighters gang you can enter, this involves men punching and kicking each other in a ring.
- Having too much beer can result your character to throw-up on the ground.
- Your sister has blood near where bandages cover her eyes.
- Various pictures depict people slaying creatures with blood depicted as well.
- One of your "trophies" is a bloody, decapitated Hob head. Another trophy is a bloody, decapitated white balverine head.
- In the game you can break the law, this involves you being able to hit other villagers, vandalise property, steal from shops, all which resolved the guards to come after you and if you don't pay the fine the guards can get very violent indeed.
Profanity

- Various expressions can be used in the game, one involves the main character to say s**t. Also a demon door can be heard saying s**t when you spell his name wrong by striking the wrong mysterious stones. Another expression allows the main character to give someone the middle finger.
- NPC's can be heard saying "ass" from time to time.
Alcohol, Drugs & Smoking

- You can drink and get drunk; likewise you can get others drunk.
Frightening & Intense Scenes

- For all of its violence and adult themes, the game takes a lighthearted, fairytale like tone,
- The quest where your character held in prison is quite disturbing.
- Some areas are quiet, grim and have suspenseful music playing in the background.
- Evil choices like murdering innocent, defenseless person give more darkness to the storyline.
- Some creatures throughout the game can seem a bit intense, especially on the Arena quest.
