In Version 1.0 of the game, there is mapping oversight in the Castle of Grief stage where the player can bypass much of that hub's various puzzles; a small gap on the bottom of a wall outside the moat blocking off the third entrance to the Effluvium map can be accessed if the player is turned into a pig and runs through it. This will bring the player many steps ahead of the hub's initial puzzle sequence, bypassing much of the Effluvium map and the entire Dungeons map entirely.
By using the intended way to pass the hub, the player would appear to Castle of Grief from Effluvium after having solved the majority of puzzles and fought the way through the Dungeons. In an updated version of Hexen, the shortcut was blocked.

In version 1.0, if the player picked up a disk of repulsion, the message on the top of the screen would display "chaos device" and if a chaos device was picked up, the message would display "disk of repulsion" displaying the incorrect name of these two items when they would be picked up. This error was corrected in later versions of the game.
