Doug's surname is given as both Bridges and Reiser.
The reflections of Sam while he's walking on top of the cars and fighting Jonathan are clearly a stunt person in a curly wig, not the real Dr. Ruth.
Near the end of the episode, Al is telling Sam the wrap up. In the background, you can plainly see the phone off the hook from the earlier struggle. Next, Annie comes into the room telling Sam/Dr. Ruth that the police are on the way. Unless Annie has a second phone line in her NY apartment (or a very rare cell phone in 1985), the line would've been 'dead' and she could not have called the police.
In the scene where Annie picks up the kitchen wall phone and threatens to call Jonathan's wife, he attacks her, grabbing the phone away from her and throwing it to the floor when he lunges at her. The receiver is NOT placed back up on the hook.
In the next scene, Sam (as Dr Ruth) tells Annie to call the police. Annie exits to the bedroom to call, (OMITTING SCENE DETAILS HERE). Sam and Al talk in the kitchen, where the phone is STILL off the hook, and then Annie re-emerges saying that the police are on their way. But since the kitchen phone wasn't hung up, it would have been impossible for Annie to have connected with the police via the bedroom phone.
