- The player is possessed by a demon. This gives him the ability to kill enemies by shooting with various guns or by using supernatural abilities such as sharp tentacles or black holes.
- The player can also summon demonic creatures called Darklings who can attack enemies in various ways or with various tools, usually with bloody results.
- The player can devour the hearts of dead enemies by using two demonic heads emerging from his shoulders.

- Plenty of strong language. The game features very frequent use of fuck, mother fucker and shit. The word cunt is used once.

- The demon possessing the main character often speaks with a scary and growling voice.

- Very dark and scary imagery throughout the game, especially in sections taking place in WW2-like Hell.
- Overall, because of the intensity, the themes it explores and the extreme violence it is recommended that this game isn't played by anyone under the age of 18.
