- In the Season Four premiere, Dexter sleeplessly struggles to balance his new family life with his dark, murderous drive for a fresh victim, and his busier-than-ever forensic career pursuing a new target, "The Trinity Killer."
- The show opens by splicing scenes of Dexter's new baby keeping him awake with a nude man (John Lithgow) grabbing a woman. The man pulls the woman into a bathtub and slices her femoral artery.
Dexter is exhausted and his household is chaotic mix of kids and noise. Rita seems particularly happy with their new suburban existence.
Debra and Anton are getting along swimmingly. They have moved in together and he has a regular gig working on a cruise ship.
Dexter completely screws the pooch in court. While testifying in a murder case he brings notes from a different case and allows defendant Benny Gomez to walk.
Debra is still looking through the list of her father's informants. She tells Francis she wants to know that he wasn't so perfect after all. Quinn is furious that Dexter let Gomez off the hook, showing him graphic pictures of the victims.
During Maria's briefing Dexter appears to have targeted Gomez for his next kill. Maria sends Quinn, Debra, Dexter and Vince to the murder from the opening. The victim, a Ms. Bell, had just been giving a promotion and Dexter points out that the crime scene was meticulously cleaned. Quinn is still giving Dexter a hard time.
Outside the crime scene Quinn talks with Christine Hill, an attractive reporter. He gives her his phone number.
At the office Debra is frazzled by how well her life is going, expecting things to take a turn for the worse. She had previously tried to set Maria up on a date but Maria passed.
Dexter is alone in his office. He processes the new crime scene and researches background on Gomez. Rita calls and has Dexter sing their baby to sleep.

Dexter finds old blood under the floor at the Bell crime scene. It doesn't match the victim and is also from the bathtub. Quinn continues to give Dexter a hard time. While Dexter gathers evidence, Agent Lundy walks into the bathroom. Lundy seems to know a little about the killer's M.O. and asks Dexter if he can swing by the station later.

The old blood Dexter collected yielded no hits. A check on the address shows that a 22-year old woman was killed there in 1977. Lundy shows up to speak with Dexter. Lundy is now retired, but is in Miami tracking down the Trinity Killer - "the one who got away." Lundy can't prove the killer exists so he is getting no help from the feds. He is convinced this new bathtub murder is the start of a new threesome of murders.
We cut to the Trinity Killer, again naked, taking a shower in scalding hot water.
Lundy and Debra speak briefly. He congratulates her on making detective.
Dexter finds a new grab spot for Gomez. He goes to Gomez' trailer park home and takes him when he answers the door.
In the boxing ring Dexter collects Gomez' blood and prepared for the kill. Just then Rita calls, needing him to pick up ear medicine for the baby. Dexter quickly kills Gomez and sloppily dismantles the body. On his way from the scene with Gomez in the trunk, Dexter falls asleep behind the wheel. He loses control of the vehicle and it flips over several times.
