- Dr Martin Ellingham is shocked to find Louisa and Aunt Joan knocking at his door before the Surgery has opened. Louisa has had a medical scare and wants Martin's reassurance that everything is fine.
- P.C. Joe Penhale's older brother Sam arrives and Martin employs him to paint the waiting room and surgery, but his lack of coordination leads the doctor to believe he is not well. Joe panics, believing the brothers have an hereditary disease but Sam's illness is actually caused by lead poisoning, as a result of his illegal side-line. Louisa gets nearer to renting a house in town, owned by an old hypochondriac whilst Martin is strangely touched when he sees her X-ray photo of the baby.—don @ minifie-1
- Martin has to put up with PC Joe Penhale's brother Sam in more ways than one. Pauline has hired Sam to paint the reception room, which Martin doesn't agree with, but he also notices that Sam sometimes loses his balance or his motor coordination. He's primarily concerned about the possibility of a hereditary disease and when hypochondriac Joe hears about it, he panics. Louisa meanwhile hopes to move into old Mr. Routledge's house. The old man, who drives Martin mad with his constant complaining, desperately wants to move into a care home, but Martin won't agree to refer him. He inadvertently facilitates the move. Martin and Louisa are barely talking and his friend, Edith Montgomery, thinks he should be returning to London. Meanwhile, Pauline thinks boyfriend Al is too passive and predictable.—garykmcd

Louisa moves in with an old man who wants to move to a nursing home but Martin refuses his request. The doctor is less than pleased when he hears the surgery needs to be re-decorated. Al Large goes to lengths in order to convince Pauline he is good enough for her.
