When Sally Tishell suggests her diagnosis of "dime of Lyme" aka Lyme Disease for the recently widowed Mrs. Selkirk to Doc Martin in the pharmacy, it turns out later to be correct at Mrs. Selkirk's farm after she was knocked down by a sheep. Considering Dr. Ellingham brushes her and her blabbering off, as usual, she quickly landed the mark long before Ol' Doc even caught a clue.
