Mycroft and Sherlock are fencing on the roof and Mycroft asks if he and Sherlock will still be dining at his new restaurant Diogenes before heads back to London. Sherlock tries to beg off but agrees.
A delivery man is making his rounds and heads into a business. The young man sitting outside hears a noise, some kind of thud. When the delivery man comes out he asks if he heard it but he didn't and the kid brushes it off. The truck drives away and we see a dead woman on top.
It turns out that she is Hayley Tyler, a transplant from Texas who was apparently a "kept woman." When Sherlock and Watson work back to figure out where on the delivery route her body landed on the truck, it is unclear whether she was pushed or fell from her apartment's balcony, but it was clear she was stabbed before it happened.
Turns out she was being kept by Ian Gale, a Steve Jobs-like figure who invented smartphones and computer gizmos and he's a gazillionaire. They meet with Gale's lawyer who says that Gale was definitely not responsible for her death. He shows them a video of Gale out of the country at the time of Hayley's death.
He is correct that he didn't do it, but incorrect that he was out of the country. Sherlock recognizes the man in the video as a stand-in, a common practice for the wealthy and powerful. However, it turns out that Gale's innocent because he's in extremely poor health, with congestive heart failure and in need of a transplant, holed up in secret in a posh hotel. Word of his condition would create shareholder panic. They learn that Hayley wasn't Gale's mistress, she was his long lost daughter, sought out to get to know her before he died. She also offered to give him rare blood transfusions. He paid her mother $2 million to disappear back in the day. He updated his will to include her and so now suspicion falls on Gale's wife, whose inheritance has gone down, and Hayley's own mother, who would get her inheritance if Hayley died.
Mycroft calls Joan and prevails upon her to make Sherlock meet with him before he leaves.
At the morgue Joan learns that the killer who stabbed her knew exactly where to do it, hinting at a medical background which Gale's wife, a former doctor, has. Bell also shows up with Hayley's mother who admits the last conversation they had was an argument over Ian Gale's intentions. She says she knows who killed her daughter: Gale's wife.
At dinner Mycroft wonders if Sherlock will ever return to London. He says he doesn't think so. Mycroft then lowers the boom. Their father is going to cut Sherlock off if he doesn't return and work with Scotland Yard and show some gratitude to him for getting him clean, hiring Joan, and getting him back on his feet. Sherlock defiantly says he doesn't care and will live in a cardboard box if he has to. Mycroft notes that Sherlock is now also responsible for Joan, whom he pays from his trust, and he needs to think about her future.
Bell and Watson interrogate Gale's wife and she says she was not upset about losing the money that would've been her inheritance. She accounts for an absence in her schedule around the time of the murder by saying she went for a walk in the park. She threatens a lawsuit if they take their accusations public. She says giving Hayley the money was her idea and notes that Hayley's share goes to Hayley's mother.
Bell picks up a lead on Hayley's on/off boyfriend and they seek him out, looking for clues at her apartment. Sherlock breaks the news to Watson about his dad and how they might have to move and take on jobs that pay. She didn't realize that's how he was paying her. He says it was just the last few months. He says he hasn't taken on a paying client in months and he thinks they both prefer working with the NYPD. He suggests both of them moving to London and working with Scotland Yard. She knows that's not what he wants. He agrees. He feels that he thrives in New York not because of who he is but who he has come to know and he's not ready to give it up. Joan says screw his father, they can find a new place to live.
They realize Hayley's boyfriend is a horse racing enthusiast and head to the track. They find him just as his horse is coming in. He says he had nothing to do with Hayley's death. He says he was picking up medication for an illness she had seemed to have for a couple of weeks. Watson follows up on seeing if Hayley was sick, but the guy who took her blood says she was fine. This points them back to the wife.
Ian Gale dies.
Gregson, Bell, Watson, and Holmes go to see Mrs. Gale, who they now think killed her husband, who she had not that long ago considered divorcing. They deduce that Mrs. Gale, the former doctor, gave Hayley an injection that not only made her sick but made sure that her blood transfusions also poisoned her husband. The lab technician lied to Watson about Hayley seeming sick because he was in cahoots with Mrs. Gale.
Mycroft goes to see Sherlock. Sherlock gives him a letter for their father. He tells Mycroft that part of recovery is structure and the structure he has with Watson is working and there may be a time to go back to London but that isn't now. In his letter he expresses gratitude and explains the situation and says it's all he can offer for now and he will let the chips fall as they may. They shake hands. Mycroft notes that Sherlock is different. He notes Mycroft is the same. Mycroft says he will be back.
Mycroft has a drink at his bar and rips up Sherlock's letter and throws it away. He calls someone in England says "my little gambit didn't work, he's decided to stay, we're going to have to come at the problem a different way."
