- The whole game is a horror FPS genre and you go around stabbing and shooting monsters and zombies. Blood is seen throughout the game during fights and smeared on walls or corpses on the ground. All monsters and enemies have blood on them too.
- Player uses a wide range of weapons to kill monsters such as a hammer, axe, knive, revolver, pistol and some rifles.
- Multiple mentions of suicide and self mutilation. The player can also view graphic and violent imagery of suicide and gore on photos in some scenes. Multiple corpses are seen.

- There are a few f words tossed around here and there. Most of it is in subtitles and when the player cutscenes. Nothing too severe.
Alcohol, Drugs & Smoking

- There is one mention of drugs throughout the game. The player can pick up a syringe that is mentioned to have morphine inside of it which replenishes the player's health when used.
- Cigarettes' and drinking are seen briefly in the game. You can find beer bottles on the floor sometimes.
Frightening & Intense Scenes

- The whole game revolves around you, the player, a disturbed young man making his way home from work and later discovers horrific scenes of mutilated and dismantled monsters and corpses. They are all made in the old source engine but they are still extremely disturbing. The whole game has an eerie atmosphere and the sounds are also frightening. Overall the game contains disturbing images, blood and gore and some jumpscares.
- There are plenty of enemies in this game, many (almost all) are zombies and heavily mutilated rotting corpses. There are also other monsters that have really creepy and disturbing skins.
- The monsters may shock a lot of people because their apperance is a whole new level to horror. Some of the monster features include: Horribly mutilated faces opened up in half, knives on arms, bodies with spikes and nails railed to them, barbed wire around torsos and arms, and chainsaw welding monsters. Most of the design can also be related to Silent Hill. Overall very disturbing and horrific.
- Most of the movements the enemies perform when they run at the player are very tweaky and fast which might disturb or frighten the player.
- The entire environment is very spooky and always nighttime and dark. You go through dark, blood smeared hospitals, creepy apartments and very claustrophobic areas. Also, the soundtrack of the game can be very terrifying at times.
- The whole game centers around depression and tries to explain it in the eyes of the main character, Simon. The whole game has an eerie, disturbing and depressing environment. The ending can upset some players.

- There is a serial killer who murders children. It's lightly implied he molested/raped the children but it's never explicitly mentioned or out right referenced.
