- Granny can kill you in many different ways. She can rip your face off with a bear trap, behead you with a guillotine, eat you, or crush you with a car. This is all partially offscreen.
- In nightmare mode, the whole house is covered with copious amounts of blood.
- Granny will strike the player across the face when found. Blood splatters on the screen.
- The player can build a shotgun used for defensive purposes against Granny. No blood is shown, but it's knocks her unconscious when shot.
- A woman hanging by her wrists can be found. Not graphic.
- A large pool of blood is seen in the garage
- Moderate Violence and blood.
- A crow can bite you if you touch him, you bleed.

- The entire game is intense and scary, as you are trapped in a house by a zombie-looking grandmother armed with a baseball bat that can appear any moment of the game, even right behind you.
- Suggested age to play: 13+, Children should be supervised by an adult while playing this game
- In an underground area of the map, a large spider with a human head attached to it can be seen. When caught, it picks you up by its legs and eats you. This is all in the players point of view. Not graphic, but terrifying.
- If you open a certain drawer in the spawning room, a saying written in blood can be seen that says "Leave this house"
- Some of the paintings may be terrifying
